High-Resolution Manometry in Delhi

Manometry records the pressure and coordination of gut muscles through a thin catheter lined with sensors. Oesophageal HRM diagnoses achalasia, spasm and weak peristalsis; anorectal HRM diagnoses dyssynergic defecation and incontinence. Dr Paliwal pioneered solid-state and 3D high-resolution manometry in Indian private practice and has performed over 3,500 studies.

Oesophageal manometry

For difficulty swallowing, non-cardiac chest pain, suspected achalasia, and before any anti-reflux surgery. A 4 mm catheter is passed through the nose; you take ten sips of water while pressures are recorded. Takes 15–20 minutes, no sedation, immediate Chicago-classification report.

Anorectal manometry

For chronic constipation, incomplete evacuation, faecal incontinence and Hirschsprung's evaluation. A small catheter with a balloon records sphincter pressures, sensation and the push manoeuvre; combined with a balloon expulsion test. Painless, 20–30 minutes.

Why high-resolution and 3D

Closely spaced sensors give a colour pressure map that reveals subtle motility patterns invisible to older systems; 3D anorectal HRM maps the sphincter in three dimensions to locate defects.

Results guide targeted treatment — pneumatic dilatation or POEM for achalasia, biofeedback for dyssynergia, medicines for spasm — rather than trial and error.

Questions about high-resolution manometry

Is manometry painful?

Mild discomfort while the catheter passes the nose or rectum, then comfortable. No sedation needed.

Where can I get manometry in Delhi?

Very few centres offer it; PSRI Hospital's motility lab under Dr Paliwal is one of the longest established in the private sector.

How should I prepare?

Fast for 6 hours for oesophageal manometry; an enema before anorectal manometry. Stop motility-altering medicines as advised.
