Grade is not the same as damage
Ultrasound 'grade' measures how much fat there is. What decides your future is stiffness — scarring — which only a FibroScan or blood scoring (FIB-4) can measure. Many grade 2–3 patients have no fibrosis at all and can reverse completely.
The 7–10% rule
Losing 7–10% of body weight reduces liver fat in nearly everyone and resolves inflammation (NASH) in most. For an 85 kg person that is 6–8 kg — achievable in 4–6 months.
What to eat
Cut sugar-sweetened drinks, fruit juice and refined carbohydrates first; they drive liver fat more than fat itself. Fill the plate with dal, vegetables, whole grains and protein. Coffee (without sugar) is protective.
Move, especially with weights
150 minutes of brisk activity a week plus two sessions of resistance training reduces liver fat even before weight falls.
When medicines help
Diabetics benefit from agents with liver benefit (pioglitazone, SGLT2 inhibitors, GLP-1 therapy). Vitamin E is used in selected non-diabetics. Newer MASH-specific drugs are emerging. Alcohol must stop.
Monitor, don't panic
